Hospitals applying for accreditation shall:

　　1.Complete the "Accreditation Application Form" (for the Word version, please send it to SHARC to review whether it is correct), and affix the official seal of the hospital after being signed by the hospital's legal representative.

　　2.Submit the Medical Institution Practice License Certificate (including original and duplicate copy) and copy of legal representative certificate, stamped with the hospital's official seal.

　　3.Scan and upload the above files to the SHARC "Hospital Accreditation and Management Platform", and send them to the SHARC's email: szharc@hku-szh.org.
